Always verify your insurance and check important details

I am in-network with the following insurance companies:

  • Caresource MyCare (opt-in only), Caresource Marketplace, and Caresource Medicaid

  • Molina Medicaid

  • Medical Mutual (check your plan as some plans may not apply)

  • Bureau of Worker's Compensation

I am NOT a Medicare provider, but I am in-network with Medical Mutual Medicare Advantage Plans (both HMO and PPO)

Check your co-pays and deductibles for both IN and OUT of Network and compare to make the decision that best meets your needs. Insurance companies direct to their IN-Network providers because they have to pay them less. You may want to go out of your network based on the counselor's specialty, experience, availability, and other factors that are important to you, such as your comfort level and trust with them. Only you can evaluate all of those factors.

 

Plan on covering your copays and deductibles or payment in full and bring a form of payment to your therapy sessions.

The Service Agreement is lengthy, but contains important information about your health record, confidentiality, communication with me, fees and cancellations, and limitations of confidentiality. The Agreement and Notice of Privacy Practices will be available to you in the Client Portal and at my office.

Bring your insurance card and method of payment to your first appointment.

 

Please bring a photo ID as many insurance companies require that you do.
