Payment Methods

  • Payment methods (cash, check, credit card, HSA, insurance)

  • I am In-Network with the following insurances:

    •  CHAMPVA

    • Humana

    • Tricare

    • Anthem

    • Medical Mutual

    • BWC

    • CareSource, Molina, and Anthem/Carelon Medicaid / MyCare and Marketplace Ohio

    • Out-Of-Network

  • I also offer Out-Of-Network services for those who do not have or want to use insurance and for services not covered by insurance (additional time, additional sessions, concierge services, travel time for site visits, written reports, involvement in legal procedings, etc.)

  • I can submit claims for both in- and out-of-network insurances (verify your benefits)
